Set in Tanga, a city on the Tanzanian Swahili coast, Dominik Mattes examines the implementation of antiretroviral HIV-treatment (ART) in the area, exploring the manifold infrastructural and social fragilities of treatment provision in public HIV clinics as well as patients’ multi-layered struggles of coming to terms with ART in their everyday lives. Based on extensive ethnographic fieldwork, the book shows that, notwithstanding the massive rollout of ART, providing treatment and living a life with HIV in settings like Tanga continue to entail social, economic, and moral challenges and long-term uncertainties, which contradict the global rhetoric of the “normalization of HIV”.
The Republic of Therapy tells the story of the global response to the HIV epidemic from the perspective of community organizers, activists, and people living with HIV in West Africa. Drawing on his experiences as a physician and anthropologist in Burkina Faso and Côte d’Ivoire, Vinh-Kim Nguyen focuses on the period between 1994, when effective antiretroviral treatments for HIV were discovered, and 2000, when the global health community acknowledged a right to treatment, making the drugs more available. During the intervening years, when antiretrovirals were scarce in Africa, triage decisions were made determining who would receive lifesaving treatment. Nguyen explains how those decisions altered social relations in West Africa. In 1994, anxious to “break the silence” and “put a face to the epidemic,” international agencies unwittingly created a market in which stories about being HIV positive could be bartered for access to limited medical resources. Being able to talk about oneself became a matter of life or death. Tracing the cultural and political logic of triage back to colonial classification systems, Nguyen shows how it persists in contemporary attempts to design, fund, and implement mass treatment programs in the developing world. He argues that as an enactment of decisions about who may live, triage constitutes a partial, mobile form of sovereignty: what might be called therapeutic sovereignty.

This book draws on research studies ranging in context from sites in sub-Saharan Africa to New York City in the USA to examine the complexity of responding to the epidemic both globally and locally. Religious systems of meaning, practices and institutions have been central to the articulation of projects for social change and inversely sometime strongly resistant to change in diverse institutional responses to HIV and AIDS. Sometimes, religious movements provided powerful forces for community mobilisation in response to the social vulnerability, economic exclusion and health problems associated with HIV. In other contexts, religious cultures have reproduced values and practices that have seriously impeded more effective approaches to mitigate the epidemic. By highlighting these complex and sometimes contradictory social processes, this book provides new insights about the potential for religious institutions to address the HIV epidemic more effectively. An ethnographic account of the South African AIDS movement and activists From the historical roots of AIDS activism in the struggle for African liberation to the everyday work of community education in Khayelitsha, Sustaining Life tells the story of how the rights-based South African AIDS movement successfully transformed public health institutions, enabled access to HIV/AIDS treatment, and sustained the lives of people living with the disease. Typical accounts of the South African epidemic have focused on the political conflict surrounding it, Theodore Powers observes, but have yet to examine the process by which the national HIV/AIDS treatment program achieved near-universal access. In Sustaining Life, Powers demonstrates the ways in which non-state actors, from caregivers to activists, worked within the state to transform policy and state-based institutions in order to improve health-based outcomes. He shows how advocates in the South African AIDS movement channeled the everyday experiences of poor and working-class people living with HIV/AIDS into tangible policy changes at varying institutional levels, revealing the primacy of local action for expanding treatment access. In his analysis of the transformation of the state health system, Powers addresses three key questions: How were the activists of the movement able to overcome an AIDS-dissident faction that was backed by government power?
