The current structure of the global food system is increasingly recognized as unsustainable. In addition to the environmental impacts of agricultural production, unequal patterns of food access and availability are contributing to non-communicable diseases in middle- and high-income countries and inadequate caloric intake and dietary diversity among the world's poorest. To this end, there have been a growing number of academic and policy initiatives aimed at advancing food system transformation, including the 2021 UN Food Systems Summit, the Sustainable Development Goals (SDGs), and several UN Climate conferences. Yet, the policy pathways for achieving a transformed food system are highly contested, and the enabling conditions for implementation are frequently absent. Furthermore, a broad range of polarizing factors affect decisions over the food system at domestic and international levels - from debates over values and (mis)information, to concerns over food self-sufficiency, corporate influence, and human rights. This volume explicitly analyses the political economy dynamics of food system transformation with contributors who span several disciplines, including economics, ecology, geography, nutrition, political science, and public policy. The chapters collectively address the range of interests, institutions, and power in the food system, the diversity of coalitions that form around food policy issues and the tactics they employ, the ways in which policies can be designed and sequenced to overcome opposition to reform, and processes of policy adaptation and learning. Drawing on original surveys, interviews, empirical modelling, and case studies from China, the European Union, Germany, Mexico, South Asia, sub-Saharan Africa, and the United States, the book touches on issues as wide ranging as repurposing agricultural subsidies, agricultural trade, biotechnology innovations, red meat consumption, sugar-sweetened beverage taxes, and much more.
How did seven low- and middle-income countries, inspired by the landmark Alma-Ata Declaration, dramatically improve citizen health by focusing on primary health care? The Alma-Ata Declaration of 1978 marked a potential turning point in global health, signaling a commitment to primary health care that could have improved the safety of air, food, water, roads, homes, and workplaces in all 180 countries that signed it. Unfortunately, progress in many countries stalled in the 1980s. The declaration was, however, embraced by a number of countries, where its implementation led to substantial improvement in citizen health. Achieving Health for All reveals how, inspired by Alma-Ata, the governments of seven countries executed comprehensive primary health care systems, deploying new cadres of community-based health workers to bring relevant services to ordinary households. Drawing on a set of narrative case studies from Bangladesh, Indonesia, Ethiopia, Nepal, Ghana, Sri Lanka, and Vietnam,the book explains how a primary health care focus succeeded in improving population health. The book also conclusively demonstrates that comprehensive, multisector, community-controlled, and population-level primary health care is a viable strategy that, against the odds, has led to sustainable, scalable good health at lower cost. Bringing together a group of experts to analyze the forty-year legacy of the Alma-Ata Declaration, Achieving Health for All is a fascinating look at the work needed to transform nations from places that make people sick to places where they stay healthy. An inspiring array of lessons learned along the way shows how readers can make policies that support the health of all people. The World Health Organization (WHO) and other global nutrition and health agencies recommend nutrition actions throughout the life-course to address malnutrition in all its forms. In this report, we examined how Nepal’s nutrition policies and programs addressed recommended nutrition actions, nutrition outcomes, and the determinants of these outcomes. We reviewed population-based surveys and administrative data systems in order to assess the data availability on nutrition actions, and on the indicators of determinants and outcomes. Our policy review identified a total of 53 recommended evidence-based nutrition actions, of which 50 nutrition actions were applicable in Nepal. Of these, 45 were addressed in the country’s nutrition policies and programs and some of the actions were only available in some districts. Nutrition actions that were not included in current policies and programs included calcium supplementation and advice on consuming calcium during pregnancy, and daily iron and folic acid (IFA) supplementation during childhood. Current policies addressed daily or intermittent IFA supplementation during preconception and food supplementation for malnourished lactating women during the postnatal period; however, there was no program to implement these actions. Nepal’s Multi-Sector Nutrition Plan (MSNP) recognized and addressed all key determinants of nutrition; it also expressed an intent to address all SDG nutrition targets for maternal, infant, and young child nutrition. Noncommunicable diseases (NCDs), however, were addressed separately by a multisectoral plan for NCDs. Our data review found that out of 45 actions that policies and programs addressed, population-based surveys contained data on only 27 actions and administrative data systems contained data on only 25 actions. Population-based surveys and administrative data sources contained no data on: food supplementation during adolescence; weight monitoring and various types of counseling during pregnancy; optimal timing (delayed) of umbilical cord clamping, support for breastfeeding and immediate skin-to-skin contact, optimal feeding of low-birth-weight infants and counseling of mothers of low-birth-weight infants on kangaroo mother care (KMC) during delivery and in the postpartum period; breastfeeding counseling, counseling on appropriate complementary feeding, counseling after growth monitoring, and inpatient management of severe acute malnutrition (SAM) during early childhood. Population-based surveys contained data on most of the indicators of immediate and underlying determinants, while administrative data systems did not have data on all indicators of immediate determinants. Data on all indicators of nutrition outcomes were available from population-based surveys. In conclusion, Nepal’s policy and program landscape for nutrition is robust, however the gaps in data availability for tracking progress on nutrition actions are much larger than the gap in policies and programs for addressing recommended actions. Future population-based surveys and modifications of administrative data systems should aim to fill the identified data gaps for nutrition actions.
With the current Second Growth and Transformation Plan (2015-2020), the Government of Ethiopia expects the agro-industrial sector to play key role in economic growth of the Country. Accordingly, the creation of Integrated Agro-Industrial Parks has been identified as one of the key mechanisms for accelerating the development of the sector and the structural transformation of agriculture. Agro-industrial parks will play a significant role in transitioning Ethiopia from an agricultural-led into an industrial-led economy. In view of that, the development of Integrated Agro-Industrial Parks has been prioritized in Ethiopia’s national development strategy and four Agro Industrial Growth Corridors have been selected for piloting the establishment of four Integrated Agro-Industrial Parks. The initiative aims at driving the structural transformation of the Ethiopian economy while reducing rural poverty and creating a better environment for increased investments in agro-processing and allied sectors. Since 1981, FAO has been a strong partner of the Government of Ethiopia towards the achievement of national food security and economic growth goals. FAO is working closely with the Ministry of Agriculture and Natural Resources to empower value chain actors and to promote inclusive, efficient and sustainable agricultural value chains. The present document is the fourth one of a series of detailed analyses of prioritized commodities, which will lead to inclusive, sustainable and stronger agricultural value chains in the Agro-Commodities Procurement Zone of the pilot Integrated Agro-Industrial Park in Central-Eastern Oromia.
This report argues that it is essential to understand the dynamics of the informal food retail sector because of its vital role in ensuring greater access to food by the urban poor. Existing policy frameworks to address food security and to govern the informal sector tend to neglect informal retail in the food system. As a result, the sector is poorly understood. The report therefore attempts to identify the characteristics of the sector that impact on its ability to address the food needs of the neighbourhoods in which the businesses are located. Although the research is focused on Cape Town, the findings are of broader relevance.
These papers address roles and issues related to social and institutional innovations and approaches in food security in Southern and Eastern Africa. They include implementation of food security policy, rural livelihood and agricultural innovation, land consolidation for food security, interdisciplinary school-based health for food security, harnessing indigenous and modern knowledge for food security, household food resource handling for food security, institutions for technological innovation, the role of land tax in food security, trade protectionism and food security, and gender-power relations in food security.
The UN Secretary-General, Kofi Annan, has instructed all UN specialized agencies and other affiliated organizations to consider how their work might advance the cause of human rights around the world. Many of these bodies have taken this call to heart, with a wide range of intergovernmental organizations (IGOs) trying to play a more active role in promoting human welfare. Power and Principle is a comparative study of how and why IGOs integrate human rights standards into their development operations. It focuses on the process of policy innovation in three UN-related IGOs: the UN Children's Fund (UNICEF,) the World Bank, and the World Health Organization (WHO). In his comprehensive analysis, Joel E. Oestreich uses case studies to demonstrate how their policies have evolved during the past two decades to reflect important human rights considerations. Drawing on interviews with dozens of staffers from IGOs, Oestreich creates a gripping narrative of the inner workings of these large bureaucracies. In each study he describes how the organization first became interested in human rights standards, how these standards were adopted as a priority, how the organization defined rights in the context of their work, and what a rights-based approach has meant in practice. The book argues that IGOs ought to be seen as capable of meaningful agency in international politics, and describes the nature of that agency. It concludes with an examination of these organizations and their ethical responsibilities as actors on the world stage.
