It has been a pleasure to comply with requests to publish this book in English. During the intervening years, there has been little to add to our views as to the best management of acetabular fractures, but an additional chapter has been incorporated comprising recent findings in our patients and slight changes in emphasis on the indications for operations. Additionally, having recognised that one of the greatest difficulties in this method of treatment lies in the pre-operative assessment of the standard radiographs, we have prepared a short series of radiographs which the reader may find advantageous for study. We are grateful to Mr. Reginald Eison who has translated and revised the French edition. JUDET Preface to the French Edition It is a long time since we first attempted surgical treatment of fractures of the acetabulum accompanied by displacement, with the aim of restoring perfect articulation. Such treatment demands an exact reconstitution of the anatomy of the acetabulum and pelvic bone. This volume comprises an account of our efforts to assess the place of open reduction and internal fixation of displaced fractures of the acetabulum. The principal aim is simple: the perfect restoration of the articular surface and the associated bony architecture.

The history of the origin and development of the new Classification of Fractures was described in the preface to the French edition. The history of the acceptance of this new concept dates back to 1986, when the Swiss Association for the Study of the Problems of Internal Fixation (AO) accepted the new Classification of Fractures. In the same year, the Trustees of the AO/ASIF Foundation, at their annual meeting in Montreux, adopted the new AO Classification as the basis for fracture classification to be used in the planned third edition of the AO/ASIF Manual. In August 1987, the French edition of "The Comprehensive Classification of Fractures of Long Bones" made its first appearance, coincident with the Congress of the International Society of Orthopaedic Surgery (SICOn in Munich. This precipitated a great deal of interest in the subject. This interest persisted, so that in February of 1988 the President of SICOT, Sir Dennis Paterson, formed a "Presidential Commission for Documentation and Evaluation" with Maurice E. Muller as Chairman.

Both the beauty and interest of fractures and fracture networks are easy to grasp, since they are abundant in nature. An example is the road from Digne to Nice in the south of France, with an impressive number and variety of such structures: the road for the most part, goes through narrow valleys with fast running streams penetrating the rock faces; erosion is favored by the Mediterranean climate, so that rocks are barely covered by meager vegetation. In this inhospitable and sterile landscape, the visitor can im mediately discover innumerable fractures in great masses which have been distorted by slow, yet powerful movements. This phenomenon can be seen for about 100 kilometers; all kinds of shapes and combinations are repre sented and can be observed either in the mountain itself or in the man-made cliffs and excavations, resulting from improvements made to the road. In the same region, close to the Turini Pass, a real large scale hydrody namic experiment is taking place -a source which is situated on the flank on the mountain, has been equiped with a tap; if the tap is open, water flows through the tap only, but when it is closed, then the side of the mountain releases water in a matter of seconds. Other outlets are also influenced by this tap, such as a water basin situated a few hundred meters away.
