Congestive heart failure poses a threat to the optimization of health in communities. The referral to a cardiac rehabilitation program post discharge from an acute care setting is often based on reimbursement by insurers and payors. Major gaps exist in the literature about the effectiveness of cardiac rehabilitation programs in helping patients sustain an improved quality of health after discharge from an acute care setting. This project explored the personal stories and feedback from heart failure patients who participated in a cardiac rehabilitation program. The patients offered information on the influence of the program in improving self-care practices and reducing their chances of readmission to a hospital. In this setting, patients were able to share best practices they engage in to manage their daily experiences. An outcome of this focus group was gaining insight on patients' perspectives on the effectiveness of cardiac rehabilitation in managing their symptoms and sustaining healthier lifestyles. This process was also instrumental in creating a model for health and healing with a pathway for patients dealing with chronic illnesses. The feedback received is also being utilized to enhance the cardiac rehabilitation program and the recommendations augmented into health care practices.
The aim of this textbook is to give guidance in prevention, lifestyle counselling and rehabilitation for cardiologists, other physicians and many different categories of health professionals in cardiac rehabilitation teams.
