Cancer Intersections is an ethnographic analysis of the complex and paradoxical efforts to access neoliberal, market-based oncological treatments in Colombia, a country where all patients are legally guaranteed access to medical services, including high-cost ones. Drawing on years of fieldwork in the city of Cali, Camilo Sanz explores the deep entanglements between medical, legal, and policy practices that share a common goal of treating and curing cancer but are hindered by bureaucratic procedures, pernicious financial interests, and class politics. Cancer Intersections shows how the interplay of these hurdles dictates the rhythm at which patients access treatment and how even in resource-rich settings, patients suffer because of market imperatives that shape how cancer treatments unfold. Through careful and measured observation, Sanz unveils how a neoliberal universal health care regime delays access to care for those reliant on public assistance, which means that some patients will start expensive treatments only after it is unlikely to change the course of the disease.

Genetic alterations in cancer, in addition to being the fundamental drivers of tumorigenesis, can give rise to a variety of metabolic adaptations that allow cancer cells to survive and proliferate in diverse tumor microenvironments. This metabolic flexibility is different from normal cellular metabolic processes and leads to heterogeneity in cancer metabolism within the same cancer type or even within the same tumor. In this book, we delve into the complexity and diversity of cancer metabolism, and highlight how understanding the heterogeneity of cancer metabolism is fundamental to the development of effective metabolism-based therapeutic strategies. The Roundtable on Environmental Health Sciences, Research, and Medicine wanted to address the link between environmental factors and the development of cancer in light of recent advances in genomics. Celebrity culture, health care, and travel attract attention in America’s media-saturated society. These worlds curiously intersect in the study of medical tourism. Although the US touts some of the finest and best-known medical facilities in the world, many jet-setting A-list celebrities, who can well afford the finest of health care, seek treatment far away from home, popularizing international sites, physicians, and procedures. These travelers, whose every move is chronicled by the media, both reflect and influence health care concerns in America. An analysis of these high-profile cases of celebrities with both life-threatening and non life-threatening conditions sheds light on the link between medical tourism and celebrity, showing how health care and entertainment intersect, and the American public responds. The Intersection of Star Culture in America and International Medical Tourism: Celebrity Treatment argues that celebrity cases and media content drive awareness of medical tourism among Americans at a time when the medical system is under intense scrutiny. By popularizing international sites for treatment, procedures not available in the US, and different approaches to patient care, media narratives present options for health care, triggering dialogue on one of America’s most important human welfare issues.
NETosis is a unique form of cell death that is characterized by the release of decondensed chromatin and granular contents to the extracellular space. The initial observation of NETosis placed the process within the context of the innate immune response to infections. Neutrophils, the most numerous leukocytes that arrive quickly at the site of an infection, were the first cell type shown to undergo extracellular trap formation. However, subsequent studies showed that other granulocytes are also capable of releasing nuclear chromatin following stimulation. The extracellular chromatin acts to immobilize microbes and prevent their dispersal in the host. Bacterial breakdown products and inflammatory stimuli induce NETosis and the release of NETs requires enzyme activities. Histones in NET chromatin become modified by peptidylarginine deiminase 4 (PAD4) and cleaved at specific sites by proteases. NETs serve for attachment of bactericidal enzymes including myeloperoxidase, leukocyte proteases, and the cathelicidin LL-37. While the benefit of NETs in an infection appears clear, NETs also figure prominently at the center of various pathologic states. Therefore, it is important for NETs to be efficiently cleared; else digestive enzymes may gain access to tissues where inflammation takes place. Persistent NET exposure at sites of inflammation may lead to a further complication: NET antigens may provoke acquired immune responses and, over time, could initiate autoimmune reactions. Recent studies identified aberrant NET synthesis and/or clearance in inflammatory/autoimmune conditions such as systemic lupus erythematosus (SLE), psoriasis, ANCA-positive vasculitis, gout and Felty’s syndrome. In the case of SLE, for example, it appears that LL-37 exposed in the NETs may be a significant trigger of type I Interferon responses in this disease. Recent evidence also implicates aberrant NET formation in the development of endothelial damage, atherosclerosis and thrombosis. NETosis is thus of interest to researchers who investigate innate immune responses, host-pathogen interactions, chronic inflammatory disorders, cell and vascular biology, biochemistry, and autoimmunity. As we approach the 10-year-anniversary of the initial discovery of NETosis, it is useful and timely to review the so far identified mechanisms and pathways of NET formation, their role in bacterial and fungal defense and their putative importance as inducers of autoimmune responses. We look forward to a rich and rigorous discussion of these and related issues that benefit from interdisciplinary approaches, collaborations and exciting discoveries.
