Excerpt from The Journal of Urology, Vol. 4 In June, 1918, we published a preliminary report (1) of a group of cases that presented as a distinguishing feature a purpuric condition confined in at least the majority of cases to the urinary tract. The onset was acute; the course marked by temperature of an irregular type, general febrile symptoms, gross hematuria, frequency of urination and dysuria. The work has been continued, more material studied and more methods employed. The complete results of this study are incorporated in this report. Although the etiology of the condition has not been established, we feel that there is sufficient evidence to justify us in assuming that we are dealing with a new disease. In 1916 Sir John Rose Bradford (2) noted the presence of a distinct hemorrhagic nephritis. His description of this type of case is so perfectly applicable to our own group of cases that we shall quote from it directly: In some instances, especially if the case is seen early, the urine may contain a large quantity of blood, so as to be obviously red to the eye... Such cases present a rather different clinical picture to the general run of cases, in that with them dropsy is neither so frequent, nor when present, so marked a feature of the case. Further, these hemorrhagic cases more frequently have pyrexia and resemble much more closely in their course infective nephritis dependent upon some microbic invasion... Excerpt from The Journal of Urology, Vol. 8 The span of twenty years covered by the life of the American Urological Association is unquestionably the most important and far-reaching that will be known in the history of urology. Born at a time when urology was branching off from the realm of general medicine and surgery - the actual division becoming necessary through the clarification of the field by precise and accurate methods of study, such study opening up problems connected with each organ of the genito-urinary tract which required for their solution the individed efforts of individuals - the American Urological Association, built on broad, democratic principles, became a vital factor in the advancement of the specialty. It has been said that the sound practices of today were but visions the day they were born. Many of the urological problems which had arisen previous to the organization of this association have since developed into sound practices. These were the visions of internists, pathologists and surgeons; for few men, at that time, had devoted their efforts solely to the study of urology. Previous to the organization of the Americal Urological Association but one large society, the French Urological Association, was in existence. A limited society, composed of a number of general surgeons especially interested in genito-urinary surgery, venereologists, and the first few urologists in this country, had been in existence in America for sixteen years, doing pioneer work of a high type. (floating kidney, pyelitis during pregnancy, &c.) do not favour the development of inflammation. For the diagnosis, moreover, it is of interest to know that the heterogeneous infection also stipulates for great distinction as regards the complication with stone formation in these complaints. Indeed, as Colombini 9 has stated, prostatitis, spermato-cystitis and deferentitis are without apparent symptoms and are Of frequent occurrence in the course Of gonorrhea. In s'everal instances we have seen the patient a few hours after an injection suffer with unbearable pains in the prostatic region. The patient cannot lie down in bed or stand on his feet. The only desire he feels is to seat himself on the closet, believing that he must defecate. The micturition is quite frequent, the last few drops are accompanied with vesical tenesmus, and at the end of the act the patient does not experience relief.
