The number of dialysis patients, and their ages, continues to increase globally. This book highlights recent advances in our understanding of sarcopenia and frailty in CKD. The prevalence of sarcopenia and frailty increases substantially as CKD progresses, and experimental studies have demonstrated the pivotal role of accumulated uremic toxin in the development of muscle wasting. Further, sarcopenia and frailty are associated with falls, bone fractures, cognitive impairment, and poor survival prognoses, especially in elderly CKD patients. The syndromes are also related to the risk of end-stage kidney disease. This book provides readers with a deeper understanding of the prevention and management of sarcopenia and frailty in CKD patients to improve their renal and overall health, and suggests directions for future basic and clinical research.
This publication is a record of some of the outstanding presentations given at the 63rd Annual Meeting of the Japanese Society for Dialysis Therapy (JSDT). The meeting was held in Kobe in 2018, celebrating the 50-year anniversary of the founding of the JSDT as a voluntary academic association. Therapy for end-stage kidney disease (ESKD) now faces vital issues besides the management of renal anemia and chronic kidney disease-mineral and bone disorder and the selection of the optimal treatment modality for each patient. These issues include the increasing incidence of malnutrition and frailty in the elderly, fragile vascular access, and the balance between medical costs and budgets. Expert members of the JSDT present 15 articles that address these issues. This book serves to promote further progress in understanding the pathogenesis of complications associated with ESKD and providing optimal therapy for patients.

A comprehensive summary of high-flux dialyzers and their properties Dialyzers today are developed with high permeability and biocompatibility in mind, even though the definition of these so- called high-flux dialyzers remains controversial. In the Japanese reimbursement system, dialyzers are divided into five types, ranging from I to V, in accordance with their clearance for beta2- microglobulin (beta2-MG). Classes IV and V (beta2-MG clearance greater or equal to 50 and 70 ml/min, respectively, at a blood flow rate of 200 ml/min) are the most common ones, used in more than 90% of Japanese dialysis patients. Membranes used in types IV and V dialyzers are called high-performance membranes (HPMs) and are characterized by an exceptionally high flux rate, permeability and biocompatibility. The book at hand covers all aspects of these HPM dialyzers, including their definitions and characteristics, clinical experiences and basic investigations. Moreover, historical HPMs and several membranes with special characteristics that are not categorized into classes IV or V are discussed.
