As the first of the nation's 78 million baby boomers begin reaching age 65 in 2011, they will face a health care workforce that is too small and woefully unprepared to meet their specific health needs. Retooling for an Aging America calls for bold initiatives starting immediately to train all health care providers in the basics of geriatric care and to prepare family members and other informal caregivers, who currently receive little or no training in how to tend to their aging loved ones. The book also recommends that Medicare, Medicaid, and other health plans pay higher rates to boost recruitment and retention of geriatric specialists and care aides. Educators and health professional groups can use Retooling for an Aging America to institute or increase formal education and training in geriatrics. Consumer groups can use the book to advocate for improving the care for older adults. Health care professional and occupational groups can use it to improve the quality of health care jobs.
The OAA was enacted to help older adults remain in their homes and communities. In FY 2008, about 5% of the nation's adults 60 and over received key aging services through Title III of the OAA, incl. meals and home-based care. Studies project large increases in the number of adults who will be eligible for services in the future amid likely gov't. budget constraints. This report determined: (1) what is known about the need for home- and community-based services like those funded by OAA and the potential unmet need for these services; (2) how have agencies used their funds, including Recovery Act funds, to meet program objectives, and (3) how gov't. and local agencies measured need and unmet need. The legislation, administration, funding, history of programs, and related acts to assist older persons have been drawn together to provide a single source of information on all aspects of the Older Americans Act of 1965. The entitlements include the purposes and implementation plans for grants to state and community programs, such as disaster relief and nutrition services; training, research and discretionary projects such as legal services or long-term care; community service employment; and grants to Indian tribes. Relevant sections of the related acts include adult education, civil rights, economic opportunity, food stamps, housing, rehabilitation, urban mass transit, etc.

Title III, Medicaid and Medicare, state, and other sources of funding provide for several types of services, including congregate and home-delivered meals, transportation, and support for caregivers. This testimony reports on ongoing work by the auditor in preparation for the reauthorization of the OAA. This is the diary of an adolescent girl diagnosed with mental illness. She seems obsessed with celebrities such as Melissa Manchester, Styx, Madonna, Barbra Streisand, and affected by the book Carrie White by Stephen King. Her writing is based on sound associations, leading to rhymes, but sometimes incongruous, and in line with a perceptual disorder. So that a statement at the beginning of a sentence may be totally contradicted by its end. Her thoughtprocessing at times is like a train that switches tracks in the midst of a trajectory. Many times however it does make sense. It is hoped that this book may forge the way to innovative approaches to the treatment of thought disorders, which is a the basis of paranoid disorders, enlisting the strength of the affected individuall to ""put the train back on the correct track,"" and use more education and less drugs.
