The primary objective of this product is to provide a brief practical guide of policy options to support national (health and labour) and workplace level policy makers to implement the evidence-based recommendations of the forthcoming WHO guideline on mental health at work. The policy brief describes the policy options required to prevent mental health conditions at work, to promote mental health at work, and to ensure people living with mental health conditions can access and effectively participate in line with the recommendations in the WHO guideline. Similar to the WHO guideline on mental health at work, the policy brief includes considerations for lower-resourced settings. The policy brief aims to increase the implementation of evidence-based mental health prevention, promotion and care for people who work or who aspire to work and are living with mental health conditions. By doing so, the product strengthens the enactment of WHO Comprehensive Mental Health Action Plan 2013-2030, WHO Global Strategy on Health Environment and Climate Change, and International Labour Organization Conventions relevant to the area of mental health, such as, but not limited to the Occupational Safety and Health Convention.

A series of reviews of mental health and work policies in selected OECD countries revealed the challenge of mental health for social and labour market outcomes and policies and the high costs of the continued stigmatisation of mental health for individuals, employers and societies. To better respond to this challenge, in early 2016 health and employment ministers from the 38 OECD countries endorsed a Recommendation of the Council on Integrated Mental Health, Skills, and Work Policy.
For a large proportion of the global population, mental health and work are integrally intertwined. Mental health is more than the absence of mental health conditions. Rather, mental health is a state of mental well-being that enables people to cope with the stresses of life, to realize their abilities, to learn well and work well, and to contribute to their communities. Mental health conditions occur irrespective of whether work has causally contributed to them. Poor mental health has a negative effect on a person’s cognitive, behavioural, emotional, social and relational well-being and functioning, their physical health, and their personal identity and well-being as related to work. A person’s capacity to participate in work can be consequently impaired through a reduction in productivity and performance, reduction in the ability to work safely, or difficulty in retaining or gaining work. Presenteeism (or lost productivity, which is where the largest financial costs lie), absenteeism and staff turnover affect both workers and employers and, in turn, the society’s economy. An estimated 15% of working-age adults have a mental disorder at any point in time. The size of the public health problem of mental health conditions is greater than the volume of investment to address it. This is the case despite international conventions calling for the protection of workers’ physical and mental health through national policies in occupational safety and health.
The increasing number of refugees, asylum seekers, and irregular migrants poses a challenge for mental health services in Europe. This review found that these groups are exposed to risk factors for mental disorders before, during, and after migration. The prevalence of psychotic, mood, and substance-use disorders in these groups varies but overall resembles that in the host populations. Refugees and asylum seekers, however, have higher rates of post-traumatic stress disorder. Poor socioeconomic conditions are associated with increased rates of depression five years after resettlement. Refugees, asylum seekers, and irregular migrants encounter barriers to accessing mental health care. Good practice for mental health care includes promoting social integration, developing outreach services, coordinating health care, providing information on entitlements and available services, and training professionals to work with these groups. These actions require resources and organizational flexibility.

Tackling mental health problems of the working-age population is a key issue for labour market and social policies in OECD countries, not just for health systems. Governments increasingly recognise that policy has a major role to play in keeping people with mental health conditions in employment or bringing those outside of the labour market into it, and in preventing mental illness. This report on New Zealand is the tenth in a series of reports looking at how broader education, health, welfare and labour market policy challenges are being tackled in a number of countries. The report is also the first one published after the endorsement of the OECD Recommendation of the Council on "Integrated Mental Health, Skills and Work Policy" and assesses New Zealand's performance against the strategic policy framework agreed by all OECD countries. The report concludes that awareness and policy thinking is well developed in New Zealand but that structural and institutional weaknesses limit the provision of timely, integrated health and employment services, with particularly disappointing outcomes for the indigenous population. Against the background of the OECD Council Recommendation, the report proposes improvements in policy development and policy implementation to make youth, workplace, health and welfare policies ready for the challenge.
