Eighteen years have passed since Sam Morgan came West from Pennsylvania and learned the perilous business of trapping in the wilderness of the Rocky Mountains. Now, in 1840, he seems to face a bleak future. The fur trade has played out and he must find other means of making a living. Sam decides to return to California with his daughter Esperanza and start a new life. The great golden land represents a bitter memory — his beloved Crow Indian wife Meadowlark died there in childbirth — but friends convince him that his destiny lies on the Pacific shore. Meadowlark's uncle, Flat Dog, his family, and Hannibal MacKye, the half-Delaware mountain man, join Sam and Esperanza for the journey west, where they hope to trade for a herd of Appaloosa horses to sell at a profit in California. At Fort Hall on the Oregon Trail, Sam and his people encounter a terrified woman, Lei Palua, who has escaped the clutches of a psychopath called Kanaka Boy, whose gang of killers and rapists has been terrifying Indian villages in the Northwest. Sam Morgan and his people take Lei Palua under their wing unaware that her one-time lover and now bloodthirsty nemesis dogs her trail, vowing to kill her and all who stand in his way. Public and political concern about the increasing prevalence of diabetes has prompted major concern about treatment of patients with the condition. Foot complications are some of the commonest causes of hospitalisation of people with diabetes and if not treated well often lead to amputation. There is evidence that 85% of these amputations can be prevented by better understanding of the problem and by multi-disciplinary teams working more effectively together. This has been recognised and NICE have recently published guidelines on diabetic foot complications as have Diabetes UK and NHS Diabetes. These have been successful in raising awareness of the problem but the local multi-disciplinary teams need clear practical advice on how to manage the foot in diabetes and deliver high quality care. With the current interest in improving outcomes for patients with foot complications this is an ideal time to make a practical evidence-based handbook available. This book will provide clear practical guidelines on how to manage all aspects of the foot in diabetes as well as an in-depth analysis of the most recent evidence. The book will be based on care pathways with algorithms for each section so it would be of practical value in any clinic in primary or secondary care. It will appeal to a wide range of health care professionals treating people with diabetes: vascular surgeons and trainees, orthopaedic surgeons, diabetes specialist nurses, podiatrists and tissue viability nurses.

Can the United States provide a health care program that offers a comprehensive package of the highest-quality health benefits to all Americans while containing health care costs? In this important book, Dr. William L. Kissick says that it cannot: no society in the world has sufficient resources to provide all the health services its population is capable of utilizing. Dr. Kissick was an active participant in the drafting of Medicare legislation in the 1960s and for the past twenty-five years has held joint positions in a medical school and a business school where he has specialized in health care management. Drawing on his long experience in the field, he discusses the dimensions of the current crisis, the financial and medical implications of alternative proposals--including the program put forth by the Clinton administration--and the requirements for long-term strategies. He argues that although there are no ideal solutions to health care reform, there are many significant programs at the regional, state, and local level that can serve as prototypes for the restructuring of the organization, financing, and delivery of health services. Dr. Kissick discusses some of these alternatives and suggests that after the federal government legislates a health care policy, it should be implemented through collaboration with state and local initiatives, for such programs have been built on an understanding of regional needs, expectations, and cultural diversity.

In this intriguing narrative, David Dary charts how American medicine has evolved since 1492, when New World settlers first began combining European remedies with the traditional practices of the native populations. It’s a story filled with colorful characters, from quacks and con artists to heroic healers and ingenious medicine men, and Dary tells it with an engaging style and an eye for the telling detail. Dary also charts the evolution of American medicine from these trial-and-error roots to its contemporary high-tech, high-cost pharmaceutical and medical industry. Packed with fascinating facts about our medical past, Frontier Medicine is an engaging and illuminating history of how our modern medical system came into being.
Diseases and injuries were major concerns for ancient Egyptians. This book, featuring some sixty-four objects from the Metropolitan Museum, discusses how both practical and magical medicine informed Egyptian art and for the first time reproduces and translates treatments described in the spectacular Edwin Smith Papyrus.
The 21st century has witnessed a complete revolution in the understanding and description of bacteria in eco- systems and microbial assemblages, and how they are regulated by complex interactions among microbes, hosts, and environments. The human organism is no longer considered a monolithic assembly of tissues, but is instead a true ecosystem composed of human cells, bacteria, fungi, algae, and viruses. As such, humans are not unlike other complex ecosystems containing microbial assemblages observed in the marine and earth environments. They all share a basic functional principle: Chemical communication is the universal language that allows such groups to properly function together. These chemical networks regulate interactions like metabolic exchange, antibiosis and symbiosis, and communication. The National Academies of Sciences, Engineering, and Medicine's Chemical Sciences Roundtable organized a series of four seminars in the autumn of 2016 to explore the current advances, opportunities, and challenges toward unveiling this "chemical dark matter" and its role in the regulation and function of different ecosystems. The first three focused on specific ecosystemsâ€"earth, marine, and humanâ€"and the last on all microbiome systems. This publication summarizes the presentations and discussions from the seminars.
