This book reframes the purpose of infrastructure from being an input to economic growth to becoming a major instrument in reducing socio-economic inequalities in both industrialized and developing countries. Drawing on global and national lessons of COVID-19 and extensive working experience in 55 countries, this book reviews infrastructure policies and performance over several decades and suggests that the “underperformance” of infrastructure could be improved by more attention to users and the demand side, and thereby contribute to overcoming many obstacles facing low-income communities around the world. This book argues that growth is not a necessary condition for sustainability or social justice, and that both are undermined by structural inequalities which reduce the income and opportunities of urban households. More focus on user needs can substantially change the distribution of benefits and the quality of living conditions of low-income people. It provides a unique theoretical and on-the-ground critique of conventional infrastructure practices while illustrating to readers the many positive experiences around the world. More infrastructure is not enough; different and better is needed. This book reviews World Bank experience in launching a 30-year program to alleviate urban poverty through some 7,000 projects, showing that many significant policy changes have led to a big improvement in global urban policies adopted by national and local governments, yet have failed to significantly address and reduce intra-urban inequalities. The audience for this book includes academics, both faculty and advanced graduate students, while also a wider public interested in the prospects for international development.

More than 50 percent of the global population resides in urban areas where land policy and infrastructure interactions facilitate economic opportunities, affect the quality of life, and influence patterns of urban development. While infrastructure is as old as cities, technological changes and public policies on taxation and regulation produce new issues worthy of analysis, ranging from megaprojects and greenhouse gas emissions to involuntary resettlement. This volume, based on the 2012 seventh annual Land Policy Conference at the Lincoln Institute, brings together economists, social scientists, urban planners, and engineers to discuss how infrastructure issues impact low-, middle-, and high-income countries. Infrastructure drives economic and social activities. For urban areas, the challenges of balancing economic growth with infrastructure development and maintenance are reflected in debates about finance, regulation, and location and about the sustainable levels of infrastructure services. Relevant sectors include energy (electricity and natural gas); telecommunications (phone lines, mobile phone service, and Internet); transportation (airports, railways, roads, waterways, and seaports); and water supply and sanitation (piped water, irrigation, and sewage collection and treatment). Recent research shows that inadequate infrastructure is associated with income inequality. This is likely linked to the delivery of infrastructure services to households, such as direct health benefits, improved access to education, and enhanced economic opportunities. Because so much infrastructure is energy intensive, efforts to reduce greenhouse gas emissions and other negative impacts must address services such as electric power and transport. Bringing the management of infrastructure up to levels of good practice has a large economic payoff, and performance levels vary dramatically between and within countries. A crucial unmet challenge is to convince policy makers and voters that large economic returns can result from improving infrastructure performance and maintenance.
This paper analyzes the extent of income inequality from a global perspective, its drivers, and what to do about it. The drivers of inequality vary widely amongst countries, with some common drivers being the skill premium associated with technical change and globalization, weakening protection for labor, and lack of financial inclusion in developing countries. We find that increasing the income share of the poor and the middle class actually increases growth while a rising income share of the top 20 percent results in lower growth—that is, when the rich get richer, benefits do not trickle down. This suggests that policies need to be country specific but should focus on raising the income share of the poor, and ensuring there is no hollowing out of the middle class. To tackle inequality, financial inclusion is imperative in emerging and developing countries while in advanced economies, policies should focus on raising human capital and skills and making tax systems more progressive.

The heterogeneity of social structures and cultural identities in many developing countries, together with traditional hierarchies, rivalries, and deep-seated biases, has perpetuated inequities. Inclusive States: Social Policy and Structural Inequalities examines the role of the state and society in addressing structural inequalities and identifies a set of policy recommendations to redress them. This book defines structural inequality as a condition arising from unequal status attributed to a category of people in relation to others, a relationship perpetuated and reinforced by unequal relations in roles, functions, decision rights, and opportunities. Inclusive states are those that direct policies to address the needs of all, that respect the rights of citizens to exercise voice and influence on which services are provided and how they are delivered, and that have an interest in strengthening the social contract with their citizens. A central focus of policy remains a concern for equity, both to level the playing field to encourage social mobility and to ensure equity in the distributional effects of policy reforms and development interventions. This book highlights two key challenges for social policy. First, policy design needs to take into account the weaknesses of basic state functions in many developing countries, since these have important ramifications for social policy outcomes. Second, in most developing countries social structures marked by historically rooted structural inequalities pose significant challenges to the provision of services and require a long-term commitment to address underlying questions and problems. This book describes some of the challenges found in different contexts and some of the ways in which these challenges can be and are being addressed. This book is part of a new series, New Frontiers in Social Policy, which examines issues and approaches to extend the boundaries of social policy beyond conventional social services toward policies and institutions that improve equality of opportunity and social justice in developing countries. In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.
Evidence indicates that actions within four main themes (early child development fair employment and decent work social protection and the living environment) are likely to have the greatest impact on the social determinants of health and health inequities. A systematic search and analysis of recommendations and policy guidelines from intergovernmental organizations and international bodies identified practical policy options for action on social determinants within these four themes. Policy options focused on early childhood education and care; child poverty; investment strategies for an inclusive economy; active labour market programmes; working conditions; social cash transfers; affordable housing; and planning and regulatory mechanisms to improve air quality and mitigate climate change. Applying combinations of these policy options alongside effective governance for health equity should enable WHO European Region Member States to reduce health inequities and synergize efforts to achieve the United Nations Sustainable Development Goals.
Public spending on infrastructure plays an important role in promoting economic growth and poverty alleviation. Empirical studies unequivocally show that under-investment in infrastructure limit economic growth. At the same time, numerous other studies have shown that investment in infrastructure can be a highly effective tool in fighting poverty reduction1. In that context, the financing of infrastructure has been a critical element of most economic growth and poverty reduction strategies in developing countries, since the start of this millennium. This book provides a comparative analysis of the aggregate and sectoral implications of higher spending on infrastructure in three very different Asian countries: China, Pakistan, and the Philippines. Particular attention is paid to the role of alternative financing mechanisms for increasing public infrastructure investment, namely distortionary and non-distortionary means of financing. The book will be of interest to scholars and policy-makers concerned with economic growth in developing countries.
