An asterisk after a name indicates that the fungus has been reported in the United States either from a field collection or as the result of an inoculation test. A question mark after the name of a fungus in the index indicates that the report is doubtful or incorrect in some way. In the alphabetical list of fungi, perfect and imperfect stages are bracketed together also, but, in addition, the imperfect stages are listed in their proper alphabetical position with cross references to their perfect stages.
