The distinguished historian of medicine Gerald Grob analyzes the post-World War II policy shift that moved many severely mentally ill patients from large state hospitals to nursing homes, families, and subsidized hotel rooms--and also, most disastrously, to the streets. On the eve of the war, public mental hospitals were the chief element in the American mental health system. Responsible for providing both treatment and care and supported by major portions of state budgets, they employed more than two-thirds of the members of the American Psychiatric Association and cared for nearly 98 percent of all institutionalized patients. This study shows how the consensus for such a program vanished, creating social problems that tragically intensified the sometimes unavoidable devastation of mental illness. Examining changes in mental health care between 1940 and 1970, Grob shows that community psychiatric and psychological services grew rapidly, while new treatments enabled many patients to lead normal lives. Acute services for the severely ill were expanded, and public hospitals, relieved of caring for large numbers of chronic or aged patients, developed into more active treatment centers. But since the main goal of the new policies was to serve a broad population, many of the most seriously ill were set adrift without even the basic necessities of life. By revealing the sources of the euphemistically designated policy of "community care," Grob points to sorely needed alternatives. Originally published in 1991. Franco Basaglia (1924-1980) was an Italian psychiatrist and activist who proposed the dismantling of psychiatric hospitals and pioneered new ideas about mental health and its treatment. Basaglia was also one of the principal proponents of Italy's Law 180, which effectively closed down large mental hospitals in Italy. His ideas and his disciples have had a decisive influence in the move away from institutional care in many parts of the world, particularly in continental Europe and South America. However, Basaglia is strikingly absent from the literature in Germanic and Anglophone psychiatry. Most of the literature about Basaglia in the last 40 years has been published by his followers and supporters and has often been largely positive, with little exploration of differing responses or possible limitations of his model. Basaglia's International Legacy: From Asylum to Community provides an overview of current thinking and the international influence of Franco Basaglia. This resource draws on the combined knowledge of clinicians, policy makers, historians, and social scientists, including a handful of Basaglia's collaborators. It provides an in-depth understanding and critical analysis of the various applications of his thinking worldwide. Organised into three broad sections, chapters examine Basaglia's work and influence in Italy; in the 'Basaglian' countries of Europe and South America; and in those countries where his influence has either been rejected or significantly modified. A total institution is defined by Goffman as a place of residence and work where a large number of like-situated, individuals, cut off from the wider society for an appreciable period of time, together lead an enclosed, formally administered round of life. Prisons serve as a clear example, providing we appreciate that what is prison-like about prisons is found in institutions whose members have broken no laws. This volume deals with total institutions in general and, mental hospitals, in particular. The main focus is, on the world of the inmate, not the world of the staff. A chief concern is to develop a sociological version of the structure of the self. Each of the essays in this book were intended to focus on the same issue--the inmate's situation in an institutional context. Each chapter approaches the central issue from a different vantage point, each introduction drawing upon a different source in sociology and having little direct relation to the other chapters. This method of presenting material may be irksome, but it allows the reader to pursue the main theme of each paper analytically and comparatively past the point that would be allowable in chapters of an integrated book. If sociological concepts are to be treated with affection, each must be traced back to where it best applies, followed from there wherever it seems to lead, and pressed to disclose the rest of its family.

In Colonizing Madness Jacqueline Leckie tells a forgotten story of silence, suffering, and transgressions in the colonial Pacific. It offers new insights into a history of Fiji by entering the Pacific Islands’ most enduring psychiatric institution—St Giles Psychiatric Hospital—established as Fiji’s Public Lunatic Asylum in 1884. Her nuanced study reveals a microcosm of Fiji’s indigenous, migrant, and colonial communities and examines how individuals and communities lived with the label of madness in an ethnically complex island society. Tracking longitudinal change from the 1880s to the present in the construction and treatment of mental disorder in Fiji, the book emphasizes the colonization of madness across and within the divides of culture, ethnicity, religion, gender, economics, and power. Colonization of madness in Fiji was forged by the entanglement of colonial institutions and cultures that reflected tensions and prejudices within homes, villages, workplaces, and churches. Mental despair was equally an outcome of the destruction and displacement wrought by migration and colonialism. Madness was further cast within the wider world of colonial psychiatry, Western biomedicine, and asylum building. One of the chapters explores medical discourse and diagnoses within colonial worlds and practices. The “community within” the asylum is a feature in Leckie’s study, with attention to patient agency to show how those labeled insane resisted diagnoses of their minds, confinement, and constraints—ranging from straitjackets to electric shock treatments to drug therapies. She argues that madness in colonial Fiji reflects dynamics between the asylum and the community, and that “reading” asylum archives sheds new light on race/ethnicity, gender, and power in colonial Fiji. Exploring the meaning of madness in Fiji, the author does not shy away from asking controversial questions about how Pacific cultures define normality and abnormality and also how communities respond. Carefully researched and clearly written, Colonizing Madness offers an engaging narrative, a superb example of an intersectional history with a broad appeal to understanding global developments in mental health. Her theses address the contradictions of current efforts to discard the asylum model and to make mental health a reality for all in postcolonial societies.
