With an increase in aging and uninsured in the U.S. population, there has been an increase in individuals seeking primary health care. In addition, many physicians have left rural communities or only stay a short time and many independent Nurse Practitioners (NPs) have started their own practices. In many instances, nurse practitioners have identified difficulties in the management aspect of their businesses as well as problems meeting the need to increase health care access and satisfaction for the clients. While many authors have discussed satisfaction and access to health care, the financial difficulties that arise in a private practice often need improvements for the enterprise to be sustainable. Approximately 30 years of comparison studies have demonstrated NPs are equitable to physicians concerning quality of care, satisfaction, and cost effectiveness. The purpose of this project is to ascertain if the NP owned Rural Health Care (RHC), a Limited Liability Company (LLC) located in rural New Mexico (NM), will meet the objectives in providing better access to care and client satisfaction by improving the business management systems and thus increase profits compared to the past performance. With more management efficiency and profits, it is expected that access to care and client satisfaction will be improved and will add to the retention of clients and ultimately, revenues. The market and risk analysis revealed the need for a NP enterprise and the study demonstrated a good outcome. After an evaluation of the positive results, a repeat study is recommended to test the vailidity and reliability of the modified survey.
Hospitals have experienced an increase in the number of patients utilizing the emergency departments (ED) for treatment of non-emergency conditions and primary care services. As a result of this increase, rural hospitals have implemented services such as rural health clinics which require that a mid-level practitioner be present at least 50% of the time as the primary care provider. It is the purpose of this project to design a proposal and business plan for the implementation of a rural health clinic as means of increasing access to primary care services for patients who may utilize the ED for primary care on non-emergent conditions. A literature review was performed for the areas of patient satisfaction, ED utilization and patient flow, and nurse practitioners as primary care providers. The identified research supported the utilization and patient flow, and nurse practitioners as primary care providers. The identified research supported the utilization of nurse practitioners as primary care providers in a rural health clinic as an alternative to using the ED for non-emergency conditions and primary care services. This project included an educational presentation to the ED nursing staff of the proposed rural hospital located near the proposed rural health clinic in regard to patient triage and identification of patients appropriate for referral to a rural health clinic. A business plan was developed to evaluate th financial feasibility of the implementation of a rural health clinic staffed by nurse practitioners. Based on the results of the literature review an business plan, a recommendation to proceed with the implementation of a rural health clinic was made to the chief executive officer (CEO) of the proposed hospital. The CEO indicated that this recommendation would be presented to the hospital's board of directors.

Straub and Walzer have assembled a well-balanced collection of articles by experts in the field of health care, beginning with two which explore the changing populations and economies of rural areas. Successive chapters explain issues such as recent developments in home patient care, cost-saving innovations, and the pros and cons of rural HMOs. Of special note are those essays which project the future of health care and provide alternative approaches to health care services such as the viability of the rural hospital in the future; progressive non-hospital options; and ways to maximize resources in the years to come. Since this detailed work investigates the major facets of the struggling rural health care system, it will prove valuable not only to health care officials, but also to health care and social science faculty, and to state and local officials whose understanding of health care issues directly affects their policy making.
"This report is all about people and using computers to connect them and their healthcare information. It is a report about how we get consumers and clinicians to use these tools, how we pay for them, and what we want the computers to do. But computers are only a tool, a means to an end. We have focused this report on computers because they seem to be the best tool--and maybe the only tool--that will allow the nation to change the way healthcare works... This report articulates a vision of an information-connected healthcare system, where consumers' privacy is protected and their convenience facilitated, where doctors and nurses have the information they need to efficiently deliver safe and effective care, where our public health and homeland security can be protected while still guarding each individual's privacy. The report recommends specific actions and broader policy objectives, all with the goal of allowing healthcare to effectively use computers and information technology. If followed, the Commission's recommendations will accelerate healthcare's transformation." [From Foreword].
