This book contains selected proceedings from the NATO Advanced Study Institute (AS I) "The Molecular Pathology of Alcoholism" held at the Hotel Il Ciocco in Tuscany during 26th August - 6th September 1990. Alcoholism remains one of the most challenging problems in medical care, with far-reaching medical, social and economic consequences. For example in the U. S. , estimates indicate that 18 million people have a serious drinking problem and that the total cost to the economy of alcohol abuse is $117 billion. Treatment of alcohol dependence and other alcohol-related disorders accounts for almost 15% of the total health bill of the United States. Despite the scale of the medical problem, biomedical research on alcoholism remains something of a 'Cinderella science'. Research funding from government and other bodies is relatively poor and the number of medical scientists working in the field remains small. The Organizing Committee for this NATO ASI, comprising Charles Lieber (New York), Timothy Peters (London), Mario Dianzani (Torino), Emanuele Albano (Torino) and Norman Palmer (perth, Director), were therefore particularly grateful to the NATO Scientific Affairs Division for their active support of this AS I, the first dealing with a topic related to alcohol abuse. We moreover hope that this support will continue. The theme of the ASI was an in depth discussion of the molecular events initiated by alcohol abuse that culminate in onset of alcohol-related disease.
This volume provides an in-depth look at the genetic influences that contribute to the development of alcoholism. Part I: Epidemiologic Studies contains five chapters that examine the various approaches employed in the study of the genetics of alcoholism. It provides a historical perspective and details all the essentials of this subject. Part II: Selective Breeding Studies highlights the results of research involving the selective breeding of rodents. This type of research has produced homogenous strains exhibiting specific behavioral responses considered significant in the development and maintenance of alcohol dependence. The studies presented in Part III: Phenotypic Studies investigate and analyze phenotypic markers that serve as correlates to the genotypic determinants of alcoholism. Through its broad scope, this volume provides for the first time a panoramic view of the knowledge available on the hereditary influences of alcoholism.

Humans are biologically programmed to seek out pleasurable experiences. Biomedicalization is seen as the natural outgrowth of continued scientific progress--a movement towards improving the quality and quantity of life through scientific inquiries using biomedical perspectives and methods. This approach carries with it the assumption that with "proper" risk assessment, detection, and treatment, our lives can be lengthened, improved, and indeed more fulfilling. Yet critics question biomedicalization's ability to deliver. There is concern about how biomedicalization can change our traditional concepts of health as we discover more conditions for which we are at risk, and health maintenance is seen as the responsibility of the individual. The purpose of the book is to describe, assess, and critique biomedicalization and its influence as a larger social trend on the health field and specifically in the area of alcohol research, policy, and programs. Chapter 1 gives a broad overview of biomedicalization. Chapter 2 lays the groundwork for a historical understanding of how medicalization and biomeidcalization have developed and are expressed in diverse fields such as aging, psychiatry/mental health, and women's health. Chapter 3 focuses in-depth on alcoholism and assesses the development and assumptions underlying the two movements that have greatly influenced the substance abuse field: the medicalization of deviance and the growth of the disease model of alcoholism. Chapter 4 discusses the origins and development of the National Institute on Alcohol Abuse and Alcoholism (NIAAA) from its inception in 1970. Chapter 5 illustrates the growing biomedicalization that has occurred in the alcohol field prior to NIAAA's movement to the National Institute of Health (NIH). Chapter 6 assesses how Sweden has handled alcohol problems and currently funds alcohol research. Chapter 7 concludes with a rationale for an expanded discourse between social scientists and biomedical researchers working on social problems, particularly alcohol issues. This volume will stimulate discussion of the processes by which social problems, and specifically alcohol issues, are framed, managed, and studied. The 39 chapters in this volume consider subjects ranging from genetics, markers, and molecular biology of alcoholism, to clinical observations and treatment. The aim is to integrate pertinent information from the fields of molecular and cell biology with view to establishing a molecular basis of alcohol use and abuse. An initial preview summarizes historical aspects of alcohol use, and subsequent chapters concern novel drugs, pharmacological aspects, gene structures, cloning, and enzymatic properties. Also contributions by "non-traditional" alcohol scientists have been included in this collection, in order to highlight possible interaction and parallels between different fields. Novel results of particular interest include up-dated summaries on receptors, enzymes, and other proteins, as well as corresponding gene structures and regulation, setting the basis for distinguishing markers and pinpointing further possible pharmacological treatments.
